I would like to take an existing wordpress theme "complexity" (see demo here) http://demo.themeblvd.com/?theme=complexity
And make a beautiful form like the example above fit into the slider window. Same questions as in form listed
I want the form to have a beautiful (web 2.0) submit button and I want the CSS elements of the form to have its own interface so I can pick a back ground color without changing the form fields
I really like this style form: (myautolicense dot com) Especially the oversized form fields.
You will be given access to the theme (with knowledge base of how the slider works)

and you can utilize the work the other freelancer did or work from scratch. I don't care as long as it works. If you have a developers license for gravity forms then that would be cool to use I think cause much of the functionality is built in.
The form should have the photo of the older people to the left side like the example, but please use your own image.
I have a the complexity theme installed and up on a site to use for this project.
This project also comes with a sidebar form which will need to be taken from this design attached. Both forms need to be functional with error messages if missing fields are submitted, along with successful submission (I would prefer some sort of ajax used here).
I had hired a freelancer to complete this, but apparently he didn't want to finish the project, so now I'm stuck hiring someone else.
