PDF Invoice With Form Fields, Digital Signature

I need a PDF Invoice template with editable fields, including digital signature functionality.
Must look very close to attached example, with the same functionality and basic design, except with my contact details, logo, etc.
Intended use is to fill out and email to clients, who can then print and fax or email back with digital signature.
I need to be able to update the form, as well, in case phone number/ fax number changes, or I need to add more text in 'description' field, etc.
Please include an example of your work that is very similar to this project in your bid.
I would like the work done within two days of selecting a bidder.
I will upload all relevant info for successful bidder, i.e. logo, business contact information, etc.
