I need someone to convert my paper forms to multi page html forms to use in the Android App Forms2Go.

It is basiclly a standard sales form with itemized fields with totals, discount, tax, etc, account info, contact information, and numerous other related fields. It will also need to have some fields or pages hidden or displayed based on the input on the main sales quote page. On the main page I want an "Accept Quote" button that will then dispaly the additional pages and fields.
I will attach the main form I tried to do that will show the main fields, (formatting is pretty good) but it does dont do anything.
Only Freelancers with Good Ratings will be considered.
All payment will be thru escrow only and not be released to any freelancer until a complete working form is delivered and accepted by us.
